New Clients
Paperwork for Adult Mental Health clients, Minor child Mental Health clients, and AODA clients only differ by several documents per section. The adult mental health packet is our standard paperwork.
Documents for minor (under age 18) mental health clients includes a Right to Authorize Treatment of a Minor, in which we ask the presenting parent to confirm that both parents are aware of the treatment, and if both parents are not aware, and the presenting parent believes that the other parent should not be made aware of the treatment, a statement as to why they should not be made aware. It is our goal to include both parents in treatment where it will benefit the child, but we understand that this is not always the case.
Substance Abuse (including Alcohol abuse and the Walworth County Drivers Safety Program), include releases of information for your therapist to talk with either your attorney, Walworth County, or both about your treatment if you need that communication to happen. Releases can also be signed to communicate with others as necessary.
Co-parenting/CBI (Child’s Best Interest Program) paperwork is the paperwork that should be completed to either participate in this program (generally court ordered), or to participate in our Re-unification program where either the court has ordered re-unification between a parent and a child, where the parent has been largely absent from the child’s life for some period of time, or where the court has not ordered such therapy, but both parents are in agreement that they both want to proceed with such therapy.
Paperwork for our “In School” Counseling program is the paperwork that a parent will need to complete in the event that their child has been referred to the in-school counseling program in one of the Walworth County schools that participate in the program. Each participating school is listed and the paperwork and school contact information for each participating school can be found in this section.
