Release of Information form is to be used when you are requesting a copy of your therapy records be released either to you or to another therapist or physician, or to an attorney for legal purposes. There may be a charge for the copy and mailing of these records, although it is generally waived when such records are being forwarded for the purpose of continuity of care with another service provider.

Contact information update is the form that we ask you to complete when there has been a change in any of your contact information (New address, New telephone number, etc)

Insurance Update form is to be completed when there has been a change in your insurance. This can be a change in companies, or can be simply a new identification number. In the case of a change in companies, please check that your therapist is still in your insurance network.
